WHAT IS FAMILY SUPPORT?
The Family Support Program is designed to assist individuals with disabilities and their families to remain together in their homes and communities.
This program is funded by state dollars and helps families purchase services, supplies and equipment.
Services provided through the program are flexible and responsive to families and their needs.
FOCUS
The Primary Focus of the Family Support Program is to support:
- Families with more than one member with a lifelong disability that is likely to continue indefinitely
- Single working parent of a family member with a lifelong disability and an individual with a lifelong disability living alone
- Person requiring multiple caregivers for a person with a disability
- Anyone else who is eligible
SUPPORTS
The Family Support Program is an assistance program of last resort. The support is available to individuals who are unable to meet their needs through other agencies, organizations or funding sources.
Some examples of supports that can be provided through this program are:
- Respite Care
- Child Care
- Homemaker Services
- Specialized equipment and supplies
- Specialized nutrition and clothing
- Maintenance and repairs of specialized equipment
- Personal assistance services
- Vehicular and/or home modifications
- Health related costs not otherwise covered
COORDINATION
Family Support Services are coordinated among the numerous agencies likely to provide resources and services to families. PDI’s Family Support Coordinator develops comprehensive, responsive and flexible supports for families as their needs evolve over time and builds on existing social networks and natural resources of support in the community.
WHO’S ELIGIBLE
An individual who receives SSI or has a disability that:
- Is attributable to a mental or physical impairment or a combination of physical and mental impairments
- This impairment is likely to continue indefinitely
- This impairment results in substantial functional limitations in three or more areas of major life activity:
- Self-care
- Receptive/Expressive language
- Learning
- Mobility
- Self direction
- Capacity for independent living
- Economic self-sufficiency
- Reflects the person’s need for special, interdisciplinary or generic cares, treatment, or other services that are of lifelong duration and must be individually planned and coordinated
